Medical Records

​We have partnered with MediCopy to fulfill Release of Information requests.  MediCopy is fully HIPAA compliant and adheres to all state and federal regulations concerning release of medical information. 

Medical Records
PATIENTS:  To request medical records, visit Patients | MediCopy.  Please provide an email address on the authorization to exepedite the process and delivery method.  Records sent via email or to another provider are complimentary.  Requests are processed within two (2) business days after receipt by MediCopy.  

If you have already submitted a request and need to check the status, please call them at 866-587-6274 or chat with them online through their website.  

Forms
MediCopy will also complete Family Medical Leave Act (FMLA) forms, short-term disability forms, cancer policy forms, and other health related forms.  Please complete the patient section of the form and email it to them at disabilityform@medicopy.net.  If you are not able to email the form you can upload it directly to their website at Form Completion | MediCopy.  Form completion cost $30 for the first form and $15 for each additional form that is turned it at the same time.  An invoice will be emailed and payments can be made online. Please allow three (3) business days AFTER the payment is received for processing.
